A nursing home facility has been fined for the death of a resident. In 2009, a 74-year-old resident died after choking on ham that was served to him.  The resident was known to be at high risk for choking and needed supervision and proper food.   A nurse’s aide was let go from the nursing home after the incident. The Illinois home was found by the state to have multiple violations.
