Daily Observation
Nursing staff should look at the patient’s skin every day. If there are places on your skin that are red, that is a sign that a bedsore could be developing. If there is a red area on your skin, find out what that red area is, what caused it and then do something about it.
Keep Skin Clean and Moist
Skin should be kept clean. Sometimes we find that the elderly become incontinent, either of urine or of bowel or in some instances of both. Urine and feces are very damaging to skin. Thus it is important to make sure that skin is clean. If your skin becomes dry, use moisturizers and take sufficient fluids. Dry skin is at greater risk for breakdown.
Turning and Repositioning
Perhaps most important, if the patient cannot move around in bed, somebody must turn and reposition the patient every two hours. The patient should be moved first on the back, then on one side, then on another side. We suggest that if one of your loved ones is in an institution, you consult with the nursing staff to make sure that turning and repositioning is actually happening. Ask the nurses to allow you to look at the records. Very often the nursing staff will document when the turning has occurred. We find sometimes that we’ll look at those records and see that the turning hasn’t occurred every two hours the way it is supposed to be.
Pressure Reducing Mattresses
Among the most important things that can be done to reduce the risk of developing a bed sore is to use a pressure-reducing mattress or a pressure-reducing bed. These are special beds that make it much less likely that a bed sore will develop.
Ask at the beginning of an institutional stay if there is some risk that you will develop a bedsore. If you are immobile or if you’re having difficulty moving around, ask, “What kind of bed is this?” And if it’s not a pressure-reducing surface, ask the doctor and nurse if this type of special bed is appropriate.
